What is Panchakarma?

The Steps

Poorva Karma

It is the preparation of body for purification.

Pradhana Karma

It is the process of Panchakarma

Pashchat Karma

It is the treatment process we do after Panchakarma.

Poorva Karma

Poorva Karma primarily involves two processes – Abhyanga (oleation) and Swedana (sudation).

In Abhyanga, the choice of oil is determined after careful consideration of Prakriti (nature + mind-body constitution), Dosha imbalances, condition of patient and season. The process of oleation can be external or internal depending on condition of patient.

Ayurveda Diagnosis

Diagnosis

Ayurveda is individualistic, which means treatment is individualised and detailed diagnosis is important to determine what is needed for patients. The diagnosis is done by qualified Ayurvedic doctors through the following steps: Darshana (sight or observation), Sparshana (touching the patient or physical examination), Prashna  (interrogation).

Through detailed consultation, doctor gets to know the Prakriti, Dosha imbalances and health condition of patients along with their mental parameters. This detailed 1-hour long diagnosis that involves extensive examination helps doctor to prescribe what is best suited for patients, in terms of medicine, treatment procedures and protocols.

Treatment Course

The treatment course will be purely individualistic and will be based on the diagnosis of a patient, in terms of medical condition and Dosha imbalances. According to Ayurveda, every disease is caused by imbalanced digestive fire (Agni). So, after consultation, doctor will describe a detailed diet to improve Agni. The cleansing processes (Panchakarma) will be determined and executed based on the condition of patient.

Snehana treatment in Ayurveda

Snehana (Internal oleation) & Abhyanga (external oleation)

Internal and external oleation is carried out in this step. The choice of oil based on constitution of patient. The primary aim is to mobilize vitiated Doshas from body to prepare for elimination. There will be a specific diet to be taken during the process. In some cases, there will also be preparatory treatment for Rukshana (drying) based on diagnosis and condition of patient.

In addition, we also do external oleation (Abhyanga) with the help of medicated oil or fat to help remove toxins. Usually, Abhyanga is followed by Swedana or sudation to flush out toxins from the body. During this process, we will be using diff Kerala treatment procedures – Uzhichil (therapeutic oil application on body), Pizhichil (pouring of oil on body), etc.

Sudation Process

The sudation process is usually done in multiple ways depending on condition of the patient. Sometimes we may also use direct application or indirect application of heat.

Podikizhi - It is a therapeutic massage using warm poultice made of herbal powder.

Takradhara – pouring medicated buttermilk on patient on a continuous stream on the head or whole body. It is great for rejuvenation of body or for skin conditions

Ela Kizhi - therapeutic massage with warm poultice made of herbs and medicinal leaves

Ksheeradhara – pouring medicated milk depending on condition of patient. This procedure is relaxing and rejuvenating

Naranga Kizhi - therapeutic massage with warm poultice made of lemon and medicinal leaves

Steam bath – this process involves placing a person in a steam chamber to induce sweat through steaming

Dhanyamla Kizhi - therapeutic massage with the help of Dhanyamla decoction – a specific herbal decoction. This procedure is done primarily for subsiding swelling and inflammation on body

Nadi Sweda –In this process, steam is applied on specific parts of the body for pain and inflammation

Njavara Kizhi - therapeutic massage using warm poultice made of a specific kind of medicinal rice and milk to nourish and rejuvenate the body

Upanaha Sweda – this process involves bandaging the body with herbal medicines

Njavara theppu – therapeutic massage using medicinal paste of milk and medicinal rice. This procedure is prescribed for patients who need more nourishment to the body

Dhrava Sweda – one of our most sought-after methods of sudation where patient is given steaming with the help of hot, bubbling water. It is effective for subsiding inflammation and for rejuvenation

Thalapothichil – this process helps to relax and calm mind with the help of medicinal paste applied on scalp

Kashayadhara – therapeutic massage by pouring medicated herbal decoction on the body

Pradhana Karma ayurvedic panchakarma treatment

Pradhana Karma

The main stage of treatment is based on detailed 1-hour consultation and diagnosis. The Panchakarma treatment is planned accordingly.

During Pradhana Karma, the patient may undergo one or more of the following treatments depending on the body:
• Vamana
• Virechana
• Nasya
• Kashaya Vasti
• Sneha Vasti

Paschat Karma

This is the final stage of the procedure for maintaining positive effects of Panchakarma treatment. During this process, the patient will be given internal medicine and a detailed course of action for the future.

The patient will again have a detailed consultation during this process where the doctor will prescribe maintenance plan after Panchakarma to ensure that the positive effects of Panchakarma are maintained after treatment. Also, the doctor will advise joining a three-month gut health program with medicines for the patient to receive complete effect of detoxification.

Supporting treatment

In addition to these procedures, the doctor may prescribe one or more of the following treatments:

Netrakalpana - treatment of eyes

 If patient suffers from disorders of eye, blur vision, glaucoma, cataract, etc., doctor may prescribe eye treatments like Sekam – washing eye with herbal decoction, Aschotanam – putting medicated eye drops, Vidalakam – application of medicated paste on eye lid, Anjanam – applying medicated Kajal, Tharpanam – application of medicated ghee in eye to improve eye health.

Karnakalpana – treatment of ear

If patient has ear conditions, such as ear pain, vertigo, imbalance, dizziness, connicus, doctor will prescribe Karna Pooranam – instilling medicated drops inside ear and Karnadhoopanam – fumigation of ear with medicated herbal fumes.

Murdhani Thailam – treatment for head and neck

If patient has problems with neck, spondylitis, hair fall, etc, one or more of the treatments will be given:
• Shirodhara- pouring of medicated oil on head in a continuous stream
• Shiropichu – Applying medicated cotton pads soaked in oil on head
• Shirovasti – immersing head in medicated oil

Oral treatment​

If patient has problems related to gums and teeth, we do treatment for mouth. One or more of the following treatments will be given:
• Gandhusham – holding medicated oil tightly in mouth
• Kabalam – Holding of medicated oil loosely in the mouth
• Application of medicated paste in the mouth

Mughalepam​

Applying herbal paste on the face depending on skin condition of the patient

Vasti​​

Holding medicated oil on prescribed times at specific areas of body which need additional care in case of inflammation, swelling or pain. Depending on the condition of patient, doctor may prescribe one or more of the following treatments:
• Kadi Vasti – holding medicated oil on the spine or lower back
• Janu Vasti – for the leg
• Griva Vasti – for the neck

Footnote: Please note that all these treatments are prescribed by the doctor purely based on the diagnosis of the patient. All the above treatments may not be given to patients at all times. But if the doctor finds it necessary, they’ll be given additional treatment apart from the ones mentioned above for additional conditions.

Supporting treatment

Medicines

Medicines given to patients are all made under Dr Rekha’s supervision in our own clinic following traditional Ayurvedic principles. Most of ingredients are freshly picked from local farms. We do not give any herbo-mineral medicines. We try not to give even Shatavari or Aswagandha in our prescription.

Internal medicines include Kashayam – medicinal decoction, Choorna – powdered medicine, Gulika – tablet, Ghritam – medicated ghee, Arishtam – herbal wine, Lehyam – herbal marmalade.

External medicines/oils are also prepared in the following elaborate protocols based on traditional Ayurvedic recipes.

Food

Our treatment addresses the root cause of diseases. 98% of the time, the root cause is Ahara and Vihara (food and lifestyle) of the patients.

So, our first line of treatment will be correcting the Ahara and Vihara. We provide a customised diet plan with each food item that is based on the digestive fire (Agni) of the patient. 80% of healing takes place through diet.

During digestion, food is broken down into to Sara (good portion) and Kitta (bad portion). Kitta goes for elimination, while Sara helps in nourishing the Dhatus which are the 7 tissue systems in the body that comprise of Rasa Dhatu (plasma), Rakta Dhatu (blood), Mamsa Dhatu (muscles), Meda Dhatu (fat), Asthi Dhatu (bone), Majja Dhatu (bone marrow), Shukra Dhatu (reproductive fluid or semen). The effective breakdown of food happens only when the Agni is balanced. Hence, it is important to have a balanced Agni aka Sama Agni to have good digestion and absorption.

Even if you eat everything good, a bad Agni will affect digestion. So, our diet plan is focused on balancing Agni so that digestion and absorption is on point.

Yoga and meditation

Hatha Yoga – Patients can join everyday yoga sessions during treatment.
